CARE - INNOVATE – Since its creation in 2009, ALIMA has treated more than 13 million patients in 15 countries, and today deploys its operations in 12 countries in Africa and Haiti. In 2022, we developed 62 medical humanitarian response projects to meet the needs of populations affected by conflict, epidemics and extreme poverty. All these projects are in support of national health authorities through nearly 511 health facilities. We work in partnership, especially with local NGOs, as soon as possible to ensure that our patients benefit from the expertise, whether at the level of their country or the rest of the world. In addition, to improve the humanitarian response, we conduct operational and clinical research projects, particularly to combat malnutrition and viral haemorrhagic fevers.

Project INI — Nutrition Integration–Vaccination
The Nutrition Integration Project–Vaccination (INI), which will be implemented in 2 countries, the Democratic Republic of the Congo and Niger, for a period of 3 years, aims to simultaneously address two of the main causes of infant mortality in sub-Saharan Africa: malnutrition and infectious diseases. These two issues feed each other: malnutrition increases vulnerability to infections, while infectious diseases worsen the nutritional status of children, creating a vicious cycle that compromises their survival and development.
To break this cycle, the INI approach integrates the distribution of preventive lipid nutritional supplements in small quantities (SQ-LNS) with routine vaccination services for children aged 6 to 23 months. This strategy simultaneously strengthens child nutrition, improves immunization coverage and reduces mortality through a single delivery platform that is more accessible to families.
Job overview
SQ-LNS are ready-to-use nutritional supplements whose effectiveness is widely demonstrated. They help to reduce infant mortality, anemia, wasting, stunting and developmental delays. Their use is recommended by the World Health Organization in contexts of food insecurity.
The INI approach is also based on encouraging results from trials in Africa. The NutriVax study in Nigeria showed that associating SQ-LNS with routine vaccination significantly increased measles immunization coverage and reduced the number of children who had not received a vaccine dose (zero-dose). Promising results are also seen in ongoing trials in Chad and Niger.
In this context, the Gates Foundation supports this new project for an operational scale-up of the INI strategy in order to Democratic Republic of the Congo and Niger, to assess the impact of this integrated approach on immunization coverage and children’s health. The project supports governments in implementing integrated delivery models, tailored to national priorities, to sustainably strengthen nutrition and immunization programmes and improve survival and development of young children.
Overall objective: to reduce infant mortality and morbidity by integrating nutrition and immunization activities in order to improve immunization coverage and nutritional status of children aged 6-23 months.
Specific objectives:
Joint administration of the SQ-LNS strategy and routine vaccinations in order to achieve a double health impact: a reduction in malnutrition and an increase in vaccination coverage.
Implement a scalable operational and financial model that can be replicated in other regions.
Strengthen government ownership and co-financing, to allow for extension without the support of external actors.
Develop a roadmap for local or regional production of SQ-LNS.
This programme aims to reach approximately 175,000 children in Niger and 185,000 children in DRC over three years (360,000 children in total).
