Job Purpose: The Health Records & Information Officer (HRIO) will be responsible for managing, collecting, analyzing, and protecting patient health information and clinical data. The incumbent will ensure seamless medical record retrieval, clinical coding accuracy, compliance with data protection laws, and timely statutory reporting to support quality patient care and hospital administration.
Key Responsibilities:
Patient Registration & Archiving: Oversee patient registration, maintain the Master Patient Index (MPI), and manage physical and electronic health records using standardized filing systems (e.g., Terminal Digit Filing).
Clinical Coding & Indexing: Assign accurate disease and procedure codes using ICD-10/ICD-11 guidelines for clinical auditing, insurance billing, and morbidity/mortality classification.
Health Data Analysis & DHIS2 Reporting: Compile, validate, and report routine hospital performance statistics (BOR, ALOS, bed turnover) and upload timely monthly statutory returns to national platforms (e.g., DHIS2).
Data Privacy & Compliance: Enforce strict patient confidentiality and health data security in compliance with data protection legislation and institutional policies.
System Management & Support: Administer daily Hospital Information System (HIS) / EHR workflows, ensuring data integrity, resolving user query issues, and executing downtime procedures during system outages.
Record Quality Audits: Conduct regular quantitative and qualitative health record audits to ensure documentation completeness, clinical clarity, and compliance.
Profile:
Education: Diploma or Bachelor’s Degree in Health Records & Information Management from a recognized institution.
Professional Registration: Valid license and active registration with the relevant Health Records Officers professional board/body.
Experience: Minimum of 2–3 years' experience working in a busy hospital environment.
Technical Skills: Proficiency in Hospital Information Systems (HIS), Electronic Health Records.
