REQUEST FOR QUOTATION (RFQ) FOR RESPIRATORY AND OXYGEN SUPPLIES AND EQUIPMENT RFQ Title: CATEGORY 2 – Respiratory and Oxygen Supplies and Equipment RFQ Number 2612-002 Issuance Date: September 14, 2026 Submission Date and Time September 28, 2026 Delivery Scope Incoterm: Delivered Duty Paid (DDP), Pact ACHIEVE Liberia Office, Apartments 301-302, Embassy Compound, 13th Street, Sinkor, Monrovia, Liberia Delivery Date: October 15, 2026 Locations: Apartment 301-302, Embassy Compound, 13th Street, Sinkor, Monrovia, Liberia. Scope: The vendor must provide on-site delivery and, for applicable equipment, installation and end-user operational training for healthcare staff. Applicable equipment must include a 12-month warranty, and the vendor must acknowledge requests for emergency technical support within 24 hours.

Any firm or individual violating these standards will be disqualified from this procurement, barred from future procurement opportunities, and may be reported to the relevant Donor. Employees and agents of Pact are strictly prohibited from asking for or accepting any money, fee, commission, credit, gift, gratuity, object of value or compensation from current or potential vendors or suppliers in exchange for or as a reward for business. Employees and agents engaging in this conduct are subject to termination and will be reported to the Donor. In addition, Pact will inform the Donor of any supplier offers of money, fee, commission, credit, gift, gratuity, object of value, or compensation to obtain business. Offerors responding to this RFQ must include the following as part of the submission: Disclose any close, familial, or financial relationships with Pact or project For example, if an offeror’s cousin is employed by the project, the offeror must state this. Disclose any family or financial relationship with other offerors submitting proposals. For example, if the offeror’s father owns a company that is submitting another proposal, the offeror must state Certify that the prices in the offer have been arrived at independently, without any consultation, communication, or agreement with any other offeror or competitor for the purpose of restricting Certify that all information in the quote and all supporting documentation are authentic and accurate.
