Plan International has worked in Zambia since 1996 and currently working in over 46 communities. It has its Country Office in Lusaka and Program Area Offices (PAs) in Central, Luapula and Eastern Provinces. The organization is guided by a Country Strategy (CS) 2022 – 2026, which outlines Plan International Zambia’s programmatic ambitions with a specific focus on SOYEE, IQE, SRHR and ECD. Gender, child protection, influencing, and climate change remain cross cutting issues. The CS has a specific focus vulnerable and excluded children particularly adolescent girls, as they are often the most marginalized and most often left behind. It is against this background that the Plan International Zambia is soliciting quotations for medical insurance scheme/cover, Group Life Assurance (GLA) and Group Personnel Accident (GPA) for members of staff for a period of one (1) year Offerors MUST quote for each complete component. Plan International Zambia reserves the right to award the services to 3 different vendors for the 3 categories advertised 1. MEDICAL INSURANCE SCHEME/COVER Benefits must be per insured life per annum. The medical insurance cover will run for an initial period of 1 year i.e. from 1 st September 2026 to 31 st August 2027. Thereafter, the contract may be renewed annually for a period not exceeding 3 years and subject to satisfactory performance. Number of primary staff is 93, spouses 59 and children: 197. Total Number of lives: 349. Area of cover: ZAMBIA Area of cover (Evacuations): Local Overall Annual Limit: ZMW 100,000 Inpatient Benefits Subject to Overall Limit, Accommodation, specialists, physicians, theatre, ward, and medicine costs ZMW 3,600 per day for private ward Subject to Overall Limit Prescribed drugs (take-out): Subject to Outpatient Limit Emergency ambulance services: Subject to Overall Limit Intensive care: Subject to Overall Limit Psychiatric hospitalization: ZMW 20,000. part of Overall Limit Internal medical appliances: Subject to Overall Limit External medical appliances: Subject to Overall Limit MRIs / CT scans: One per body region per annum subject to Overall Limit Outpatient Benefits Overall Outpatient Limit: ZMW 15,000 Psychosocial Counselling (therapy sessions and treatment): ZMW 15 000 pulled from Overall Limit Consultations and specialist: Subject to Outpatient Limit Prescribed medication: Subject to OPL Diagnostic tests: Subject to OPL Pathology and basic radiology: Subject to OPL Non-surgical procedures: Subject to OPL Auxiliary services such as physiotherapy, chiropractic’s and s speech therapy : Subject to OPL Chronic Conditions Benefit : Subject to Outpatient Limit Chronic Medication: Subject to Outpatient Limit Chronic condition consultations: Subject to registration on chronic disease management programme Basic Dentistry: ZMW 3 500 Emergency/ Specialized Dental: ZMW 10 000 Optical benefits: I x eye test per insured person per year.Frames and lenses (including
