The Role As a TrinityCare Based Care Provider You will deliver tailored healthcare and follow-up services to patients within their homes, ensuring care is customized to meet their specific needs. You could potentially assist patients who are convalescing from illness, surgery, hospitalization, or childbirth, as well as elderly individuals, those managing chronic conditions, and people needing continuous care in their homes. Your responsibilities may include: Post-hospitalisation aftercare Post-surgical recovery support Nursing care at home Elderly and senior care Providing support with personal care routines and activities of daily living forms the core of this role. Professional medication reminders and support, delivered within your authorized scope of practice, are essential responsibilities in this role. Monitoring and documenting patients’ vital signs with precision, including blood pressure, pulse, temperature, and respiratory rate, is a key responsibility of this role. Candidates must possess a valid nursing license and a minimum of two years of clinical experience in a healthcare setting. Proficiency in using electronic health record systems and adherence to infection control protocols are essential. Additionally, strong communication skills and the ability to work effectively in a fast-paced environment are required. Wound care services will be administered by appropriately qualified personnel. Mobility and recovery support Assistance with rehabilitation programmes Maternal and postnatal support Experienced professionals are sought to provide specialized support for newborns, ensuring all care is delivered by individuals with the appropriate qualifications. Chronic condition support We provide support in implementing prescribed care plans to ensure comprehensive patient care. Patient observation and reporting You may be required to escort patients to medical appointments as needed. Responsible for ensuring the precise documentation of care provided and visits conducted.
