PIH recherche un médecin OB/GYN expérimenté pour travailler au Maternal Center of Excellence (MCOE) au sein du Koidu Government Hospital (KGH) dans le district de Kono. Il/elle assurera des soins de haute qualité en santé reproductive et maternelle par le biais d'interventions cliniques pour les patients fréquentant le MCOE. Il/elle travaillera en étroite collaboration avec le ministère de la Santé de la Sierra Leone (MoH) pour l'élaboration et la mise en œuvre de plans de travail, de programmes de formation médicale et d'initiatives visant à améliorer la qualité des soins maternels et néonatals.

Soins cliniques, supervision et accompagnement :

  • Assurer le rôle de clinicien senior du MCOE au sein du KGH, en se concentrant sur la prestation des plus hauts standards de soins dans les services d'hospitalisation et de consultation externe de la maternité.

  • Assurer l'accompagnement et le mentorat des médecins résidents, médecins généralistes, infirmières, sages-femmes et agents de santé communautaires (CHO) de PIH et du MoH dans les domaines de la santé reproductive et maternelle.

  • Fournir des soins prénatals de haute qualité aux femmes fréquentant la clinique à haut risque. Diriger et gérer le travail et l'accouchement, en réalisant à la fois des accouchements normaux par voie basse et des interventions opératoires (ex : césariennes, accouchements par ventouse/forceps). Collaborer avec l'équipe multidisciplinaire de la maternité pour assurer la couverture clinique de tous les services du MCOE et une couverture 24h/24 pour les urgences.

  • Assurer des soins cliniques directs et prendre en charge les cas OB/GYN conformément aux directives cliniques locales, nationales et internationales, y compris la couverture chirurgicale pour les cas obstétricaux et gynécologiques. Contribuer à l'élaboration de protocoles cliniques pour le MCOE afin de garantir des soins de haute qualité.

  • Effectuer des visites quotidiennes au MCOE avec les résidents, médecins généralistes, CHO chirurgicaux, sages-femmes et infirmières.

  • Organiser des sessions cliniques (incluant des cas cliniques, morbidité-mortalité, revue de la littérature, etc.) avec les résidents, médecins généralistes, étudiants et autres cadres travaillant au MCOE pour maintenir le niveau du MCOE en tant que pôle de formation.

  • Superviser la clinique de gynécologie, en offrant une large gamme de services gynécologiques, notamment les fibromes utérins, l'endométriose, l'infertilité, le dépistage du cancer du col de l'utérus, l'oncologie gynécologique et les consultations externes de planification familiale. Réaliser des chirurgies gynécologiques mineures et majeures, y compris les myomectomies, hystérectomies, laparoscopies, hystéroscopies et cystectomies.

  • Développer et mettre en œuvre des protocoles SGBV (violences sexuelles et basées sur le genre).

  • Participer et diriger en tant que clinicien senior aux activités de sensibilisation communautaire pour les activités RMNCAH telles que le dépistage des grossesses à haut risque, les échographies obstétricales, le dépistage du cancer du col de l'utérus, la planification familiale et tout autre service inclus dans le programme RMNCAH visant à réduire la morbidité et la mortalité maternelles dans la communauté.

Formation clinique :

  • Assurer l'enseignement clinique, le mentorat et le renforcement des capacités des cliniciens travaillant au MCOE, dans les cliniques externes et les cliniques communautaires en dirigeant et en participant à des activités telles que les visites d'enseignement, l'élaboration et la mise en œuvre de programmes de formation, les examens de cas et d'autres activités.

  • Assurer la formation chirurgicale en OB/Gyn, le mentorat et la supervision des résidents en OBGYN, des médecins généralistes, des CHO chirurgicaux, des chirurgiens généralistes et des stagiaires en chirurgie pour le développement professionnel, en soutenant le développement des compétences en OB/Gyn dans le cadre du programme de formation chirurgicale du West Africa Postgraduate College.

  • Participer activement à l'obtention de l'accréditation pour la formation de membre et de fellow du West African College of Surgeons en obstétrique et gynécologie.

  • Aider et collaborer à l'élaboration, à la mise en œuvre et à la planification des programmes de formation des infirmières, sages-femmes, CHO et médecins.

  • Assurer la formation au chevet du patient pour tous les cadres du personnel.

Administration :

  • Assister aux réunions cliniques hebdomadaires au KGH.

  • Diriger le rapport matinal du MCOE.

  • Travailler en étroite collaboration avec les directeurs médicaux et infirmiers, les directeurs médicaux et infirmiers RMNCAH, le directeur de médecine générale et la direction de PIH SL pour la planification stratégique et la budgétisation.

  • Travailler avec l'équipe de direction et l'équipe des programmes cliniques RMNCAH pour contribuer aux propositions de subventions et aux rapports destinés aux donateurs actuels et potentiels.

Leadership PIH :

Formation et expérience professionnelle :

  • MD ou formation médicale équivalente et certification de spécialiste en obstétrique et gynécologie.

  • Minimum de 3 à 5 ans d'expérience clinique depuis l'inscription, avec au moins 6 mois dans un établissement de santé à faibles ressources similaire à la Sierra Leone.

  • Expérience et intérêt pour la formation clinique, l'enseignement et le mentorat des médecins généralistes et des programmes de résidence.

  • Une expérience en oncologie gynécologique avancée et/ou en programmes de planification familiale est un atout.

  • Une expérience clinique et en gestion de programme en prévention de la transmission mère-enfant du VIH (PTME), hépatite B et maladies non transmissibles est fortement souhaitée.

  • Un engagement fort en faveur de la justice sociale et de la santé en tant que droit humain est essentiel dans tous les aspects du leadership et de la gestion.

  • Une expérience en santé publique et/ou en gestion de programme est un plus.

  • La maîtrise de l'anglais et du krio est un atout.

  • Compétences exemplaires en communication interpersonnelle, verbale et écrite, nécessaires pour interagir efficacement avec la haute direction, les partenaires et les cliniciens sur des questions politiques et confidentielles sensibles. Capacité à collaborer efficacement avec un personnel culturellement diversifié à travers les départements et les pays.

PIH is seeking an experienced OB/GYN physician to work in the Maternal center of Excellence (MCOE) within Koidu Government Hospital (KGH) in Kono District: He/she will deliver high standards of care in Reproductive, maternal health care through clinical interventions for patients attending MCOE.

Formation / Diplômes

He/she will work closely with the Sierra Leone Ministry of health (MoH) on the development and implementation of work plans, medical education programs and initiatives to improve the quality of maternal and newborn care. This position has an anticipated assignment length of a minimum of 12 months. This is an international position.

Responsabilités

Essential Duties and Responsibilities: Clinical Care, Supervision and Accompaniment Serve as senior clinical staff of the MCOE within KGH, focused on the delivery of the highest standards of care in the inpatient and outpatient departments of the maternity. To provide accompaniment and mentorship, for resident doctors, medical officers, nurses, midwives and Community Health Officers (CHOs) from both PIH and MOH, in the areas of reproductive and maternal health. To provide high-quality prenatal care to women attending the high-risk clinic. Lead and manage labor and delivery, performing both normal vaginal deliveries and operative interventions (e.g., Cesarean sections, vacuum/forceps-assisted deliveries). Collaborate with the multidisciplinary maternity team to provide clinical coverage for all the services in the MCOE and 24 hours coverage for emergencies Provide direct clinical care and manage OB/GYN cases according to local, national and international clinical guidelines, including providing surgical coverage for obstetric and gynecological cases. To contribute to the development of clinical protocols for the MCOE to deliver high standards of care. Complete daily rounds in the MCOE with residents, medical officers, surgical CHOs, midwives, and nursing staff. Organize clinical sessions (including clinical cases, morbidity-mortality, literature review, etc) with residents, medical officers, students and other cadres working at the MCOE to maintain the level of the MCOE as a Training Hub Training Hub for the MCOE Oversee the gynecology clinic, providing a wide range of gynecological services, including uterine fibroids, endometriosis, infertility, Cervical cancer screening, gynecology oncology, and family planning out-patient clinics. Perform minor and major gynaecological surgeries, including myomectomies, hysterectomies, laparoscopies, hysteroscopies, and cystectomies. Develop and implement SGBV protocols Participate and lead as senior clinician in community outreach activities for RMNCAH activities such as high-risk pregnancy screening, obstetric scans, cervical cancer screening, family planning and any other service included in the RMNCAH program planned to reduce maternal morbidity and mortality in the community. Clinical Education: Perform clinical teaching, mentorship and capacity building of clinicians working at the MCOE, outpatient clinics, and community clinics through leading and participating in activities including teaching rounds, curriculum and training development and implementation, case reviews, and other activities

Provide Ob/Gyn surgical training, mentorship, and supervision for OBGYN residents, medical officers, surgical CHOs, general surgeons, surgical trainees, for professional development, giving support for the development of Ob/Gyn skills as part of the West Africa Postgraduate College surgical training program. Participate actively in the achievement of accreditation for membership and fellowship training of the West African College of Surgeons in obstetrics and gynecology. Assist with and collaborate on curriculum building, implementation, and scheduling of nursing, midwifery, CHO, and physician training activities Provide bedside training and education for all cadres of staff Administration: Attend weekly clinical meetings at KGH Lead MCOE morning report Work closely with the Medical and Nursing Directors, RMNCAH Medical and Nursing Directors, General medicine director and PIH SL Leadership on strategic planning and budgeting Work with the leadership team and RMNCAH Clinical Programs team to contribute to grant proposals and reports to current and potential donors PIH Leadership: Lead by example, embodying the values of PIH, including doing whatever it takes to provide the highest level of service to our patients; treating every member of our organization and community with equal respect and dignity; fostering a sense of generosity and teamwork; and working patiently, yet persistently, to accompany our government partners to achieve our shared goals. Education and Work Experience Requirements: MD or equivalent medical training and board certification as a specialist as a specialist in Obstetrics and Gynecology A minimum of 3-5 years of clinical experience since registration with at least 6-months in a low-resource health care setting similar to Sierra Leone Experience and interest in providing clinical education, training, and mentorship to medical officers and residency programs Experience in advanced gynecological oncology and/or family planning programs is an asset Prevention of mother-to-child transmission of HIV (PMTCT), Hepatitis B and non-communicable diseases: clinical and program management experience strongly preferred A strong commitment to social justice and health as a human right is essential in all aspects of the leadership and management Experience in public health and/or program management is a plus Proficiency level of English and Krio is an asset Exemplary interpersonal communication skills, verbal and written, required to effectively engaging with senior leadership, partners, and clinicians on sensitive political and confidential issues.
